If you have the non drive side off remove the b.b cup from the non drive side and then you can with a bit of fingure fiddling unscrew the b.b from the frame. You are now free to work with out a big awkward frame in the way. Now you can just get a large hammer and punch to bang out the b.b spindle from the crank.
