As one of the world's clumsiest eedjits this could take a while but below are some of my favourites:
Broken wrist trying to backside flip the stairs on the southbank (Oh those crazy skateboarding days!).
The end bit of bone on my right elbow is chipped off and slides up and down my arm after years of skating.
House brick in the mouth resulting in 4 hours of surgery (local bastard anasthetic) as the dentist extracted the thousands of shards of my 2 smashed front teeth.
16 fratures in each knee after getting run over by a mk2 Cavalier. I actually walked off picking up my lemon polos first, it hurt a bit so I sat down.
Wrists, arms, legs, nose, fingers, toes....
