Assuming Shimano haven't changed the setup from what they've used forever, there should be a 2mm allen grub screw biting on the pivot pin from underneath. They're easy to strip when the threads on the screw have had a chance to rust inside the lever body... I've usually had success with a squirt of CRC or whatever, and sometimes also wrapping the allen key in a layer or two of foil, twisting it off over the end. You could also try jamming in the appropriate sized torx bit, although it might be a bit small for that trick to work.
Had this pivot move left and right under shifting with 7100 and Grx 600. Left one can be stopped by running more cable tension, right not as it will try to change gear. Pivot can be pushed side to side with force, doesn't affect shifting but can be felt as a crack/click through hood. Anyone else?
