It's hard to see how that could happen at that junction unless the driver wasn't looking.

BBC, disappointingly no reporting, just more repetition of the police press release.

No point in speculating, even on something as simple as whether a driver was looking or not. There are lots of ways in which this could have happened, we simply don't know.
