This reminds me of that box on the old forms that you had to fill in for health care in Europe, before EHIC cards. (E111 was it?)
There was a box that was titled something along the lines of "This box is solely for the use in the event of injuries sustained by self employed farmers whilst working abroad."
You just wonder what particular set of legalities, lobbying and negotiation led to there being a box just for those circumstances.
