Can anyone recommend an off the peg frame (current or something I can keep an eye out for second hand) that has a relatively large stack , fits 650b wheels with a tyre around 42mm?

Surly bridge club? or a croix de fer would do 650x42 no worries, but less stack

There’s a mtb ride I do that that has a climb that rises 1000 feet over 1000 metres and I often wish it were stretched out a bit. I don’t know that stretching it out to 5 ½ miles sounds much better though!

The bridge club is not as low as Surly normally is. It's designed for 27.5x2.4" tyres though so not what you're looking for anyways

just about functional though. i chose to assume that 42mm is not the upper limit on tyre size, unless this bike is being built around a specific pair of tyres with sentimental value
edit- it could be a bike fit thing, but then i would think shorter cranks would be easier to find than a high stack bike for small 650 tyres

650bx42 will fit in 26" bikes too, so you could think about converting an old mtb
