How is the cable clamped at both ends? I want to do this, but all droppers I've come across have the ferrule at the dropper end and the lever end held by a grub screw...but here you need the ferrule at the shifter end...is it a particular dropper that has the cable the other way around or something else?

Yes- the (defunct) ratchet is in the bin, so the shift lever just pulls cable when you move it and then returns to a resting position when you release it, just as a dropper lever does
