Spare tube and CO2 is the quickest unless you are tubeless. No point trying to faff with fixing a hole in a tube. I didn't carry anything else for an Olympic as I was very confident in how my bike was put together. Nothing else could have gone wrong that I would have been able to fix with a multitool.
Worst mechanical I have seen was a guy at a 70.3 last year who's NDS crank fell off whilst running out of transition. Luckily the mechanics there were lenient with the rules and helped him out.
