    In the US and UK, if helmets aren't worn the leading cause of death is/was head injury. When they are worn, leading cause of death is chest injury, i.e. ribs puncturing organs, internal bleeding etc.

    In the warm south of the US where people often ride in a t shirt or a denim vest, helmet compulsion led to an increase of patients who survived a big crash but had massive tissue loss from sliding down the road. They'd need a very long hospital stay with lots of skin grafts, and often they had no health insurance. This was too expensive for a number of states, so they repealed the helmet law for riders who carried proof of health insurance.
