Do you have photoshop?
If so, you can open the PDF in PS and add text where necessary then save as...
It's a bit of a pain placing / sizing the text etc. but if you already have the software it's free. -
latest version of Firefox browser claims to do this (have not tried it myself yet)

It's been about 10 years since I had a Mac but Preview used to let you open and annotate pdfs. It's about the only thing I miss. I assume it still exists?

I use DocHub, which is an app through Google. It's really really simple, and even better, it's free.

Late to the party, but Preview that ships with Mac OS can do this.
Load the form in Preview, then tools -> annotate. It's a bit lumpy but you can fill out the form and sign it in Preview.
