Late to the party, but Preview that ships with Mac OS can do this.
Load the form in Preview, then tools -> annotate. It's a bit lumpy but you can fill out the form and sign it in Preview.
Filling in pdf forms - my writing is fucking terrible so I would rather type to fill them in.
