Drill, obviously. There are a number of things you can use as spacers (valve nuts, recessed brake bolts etc...).

PDW has a bridge that fits.
Rear Hanger for Braze-ons Under Brake Bridge -
For a while I just bent that support 90 degrees and screwed it in. Later I drilled (chromoplastics, though).
