A large Pelago commuter rack, if I'm not mistaken.
I have a similar setup - albeit with a larger Wald on my dadbike.
It's not very light, but excellent quality and very sturdy (made of stainless steel)
It does not - at least not without excessive amounts of faffing/bodging.
It mounts to the fork crown and dropout area with the supplied mounts.
I did have it setup with cantis at first, but there was a bit of interference with the straddle cable, so I switched to v-brakes.
