As I was putting the grease on, to a part that I want to keep rock-solid still, I thought you might perhaps have been pulling my leg, but I did not hear a squeak out of it after greasing, on a 33 km circuit. Thank you!
I also changed the quick release bolt to one with a greater surface area gripping the dérailleur hanger from one with a small surface like that on the right in the image below to one with a larger surface area like that on the left in the same Sheldon Brown image (though both have plastic covers).

I installed a new Miche Primato square taper BB on an old frame and it's producing a click with every crank revolution, only under moderate/high load. I've checked through the more likely culprits on this list.
Removing the BB I noticed the NDS bearing is a fraction out of alignment with the spindle. I was hopeful that with sufficient torque the BB cup would help straighten it up (wishful thinking). Have I just got a dud BB?

Is it a sealed unit? Then yes, it's a dud.
If the central shield and cups are loose, then you should be able to remove the bearing from the spindle and refit.
Tightening the cups won't work as aluminium is softer than steel.
Of course, riding around with it like that may have fucked it anyway.
Serves you right for buying Miche (pronounced "mee-kay", as in "mouse") instead of a Shimano UN55.

Cheers - it's a loose central shield, but I'll see if I can swap it rather than faff about removing bearings.
I know these BBs have a bad rep, but the last one lasted for 5+ years of foul weather commuting so I was hoping for more of the same. -

Swapping it out is probably for the best.
If you've fitted it and tried screwing the cup down to sort it out, then the bearing is probably fast on the spindle: you'd need to be striking the outer race of the bearing to remove it, which will likely fuck it, if it isn't already.

In the interests of completeness:
- Miche BB replaced with Shimano UN55. Resolved the tight spot due to bearing misalignment but still clicking.
- After much searching I found a crack under the head of one chainring bolt. New bolt - clicking noise is gone.

OG Bowman Palace clicking like a MF whenever out of the saddle at bottom of DS pedal stroke.
Tighten chainring bolts = still clicky
Replaced 3yo DA BB86 press fit BB = still clicky
Inspect ancient Shimano 6700 crankset and discover worrying cracks radiating from inside of spider. Replace crankarms = still clicky
Drive side 6700 pedal body has been wobbling for ages so replace with brand new axle unit = still clicky
Endgame losing my temper wanting to bin the bike I check the derailleur hanger bolts and find one of the two bolts has a quarter turn of play in it. Tighten = SILENCE
TLDR = hanger retention bolts need some Loctite or you will spend £100

Creak from NDS crank when pedalling. Not when very soft pedalling or stamping on pedals.
Any thoughts?!

Check everything and tighten it, obviously, but are you wearing cycling shoes?
I found a massive creak that sounded like impending bottom bracket/chainset/frame collapse but was actually my cycling shoe, flexing.

If you have ridden your bike over rough ground with it’s headset loose then you might have ovalised you headtube. Shim or glue interface between hs cups and htube

Horrible creaking and clicking bottom bracket was solved by removing Shimano Alfine cranks, cleaning the axle and wiping down all exposed surfaces.
Bottom bracket cups were removed and bottom bracket wiped down.
Both pedals were removed.BB threads were greased with copperslip and BB cups re-installed.
Chainset reinistalled and left-hand crank was reinstalled correctly.
Pedal threads greased with copperslip and reinstalled.As an aside, rear axle nuts (they're proper tracknuts) were greased, so captive washers turned easily on the nut. Axle ends given a dob of grease and the axle nuts tightened up correctly.
Result: silence!

freehub bearings giving me the worst creak i've yet encountered on one of my bikes
started out as a single tick / click a month or two back
sounded like a pedal or bb creak, once per rotation has moved on to me thinking the frame was about to kark it this morning, proper horror movie rusty door hinge type sound on each rotationgood sign of this is pedals moving like you're riding fixed when you are pushing the bike, with the bearing causing drag

I am still getting a little bit of creaking, so I am beginning to wonder if the carbon frame is just old (which it is 6 years old) and that gradually it has become more sensitive to micro-movements.
