Have there been incidents of pro's losing control, crashing, etc when in the super tuck position?

Not that I recall? Most complaints I see have been about club riders copying the pros and causing issues.

No. That's why the UCI is concerned about it. Because it's totally fucking irrelevant, but distracts from the appalling job they are doing dealing with genuine safety issues.
