The bathroom at my parents place is a double fail on this - towl rad right next to the loo and shower cubicle opens on side opposite towl rad. Probably because having it open where you get access to the rad would mean the shower cubicle opening on to the loo.

We literally designed our bathrooms so that towel rail is next to the shower door in each room, I am that forgetful about it otherwise.
