Thinking about it, if you're doing a data transformation with a view to exporting to csv to import in a particular way, I can see how they would get into a really nasty mess like that.
That was my thought: either that or importing some massive data dump in csv format.
Radio just had that it was an older Excel format that meant that more than 1 thousand something tests imported in one go would fail.
So nothing to do with the 2^14 (XFD) column limit.
