Once you've added some game time go to your account and there should be a "manage" or "upgrade" button, I can't remember exactly what it said but clicking it takes you to the Ultimate page where you can upgrade for £1. https://account.microsoft.com/services/

Basically yeah. I already had the TunnelBear chrome extension so just used that, the free version is good enough.
I bought three of these

but realised I can't redeem the third yet as there's a 36 month max and I just renewed for this month, so next month I should be able to redeem the final 12 months.
Within a couple of minutes they'd emailed to confirm I wanted the Brazil one, offered others if I paid the difference. I said it's fine, got an email with the 3 codes back straight away.
Turn on vpn, go to redeem.microsoft.com, enter code. Done.
