Interesting... I've been searching for a Defy in Medium for a few days and just seen this.

How does that disc brake hydro adapter thingy work? Looks like it uses normal cable brake levers to actuate a hydraulic converter (similar to that thing Hope produced years ago)?

Pretty much. It's a slightly more evolved and less complicated unit than the Hope from back when (I hated servicing those).

I've not ridden this one as it's not mine, but tried the whole range at work when we had them, and it rides surprisingly well. Less aggressive TCR.
