could you fit that nitto M18 rack to a carbon fork? the connectors/clips look like they might not be big enough : /
wouldn't be carrying any loads beyond the odd pizza and/or six pack really

It comes with two sizes. But will depend on your fork diameter, that looks to be the smaller. I have it if you want me to check the sizes.

Yep just a couple of layers of electrical tap on the cage so it a bit padded, and then the hose clamps are these USB things from King Cage to hang a bottle underneath. Holds both without issues.

You can get tandem cranks with a ratchet mechanism (like a freehub).
In this case could be hope trials cranks set up like that?

I've had my Kepler going on 3 years now, and you're right - never going to be a dream bike but I've probably done more miles on it that anything else I've done in that time.
Perhaps that makes it my favourite bike, in that sense. If I were to get a custom frame, I'd probably use the geometry as a starting point. @Klar has one as well. She's got a sticker than sums it up well.
