The thread is worn out and damaged, hence the left crank fell off on it's own accord whilst cycling. I took this into a cycle shop and the worker upon inspection said the thread is damaged because it would not slot back in.
It's hard to see from the photos but possibly could be persuaded back on and then torque up the bolts correctly and all good?
This was attempted but fell right off after the bolts were tightened with the allen key. The crank does not stay on at all anymore
