Start with the simplest question. Are your limit screws correctly set? Can you manually push the cage all the way to the lowest gear with your hand?
I’ve found in the past that cable tension in SRAM is important. Too much and it seems to disengage something in the lever and not hold the derailleur in position.

Is this a new frame or have you set up indexing fine on it before?

No, can’t push the rear mech to the lowest gear manually. Tension in the DT cable is very tight. Have tried to slacken off the tension in the RD.

Use frame, but all seems square - no bent derailleur or anything....

Sounds like the limit screws aren’t set correctly at the moment.
Unscrew the cable pinch bolt, set the limit screws then re-set the pinch bolt for the tension.
Park tools have a good how to on the derailleur set up.

Has anyone encountered this before?
I’m indexing Force 22 and I cant get it to shift to the two lowest gears (in little and big ring) on the cassette.
When in the biggest ring the guide jockey isn’t engaging with the chain. Tried the B tension screw no luck.
What am I doing wrong?
