Agree forks are different matter. Has anyone died or been injured due to poor tolerances on the bb shell, causing creaking?

I can feel like committing murder when on a club ride all the bikes are purring like a swarm of mechanical bees except the one person whose bb never gets serviced, and week in week out it disrupts the harmony with its creaks and clunks

BB shell I don't know. But crank failure which I guess could be exacerbated by it, definitely. Anyway I've been steering clear of Cannondale forever you'll never catch me on one.
