Pack some rubble in, top with mortar then I'd expect you can just use tile adhesive and grout directly on that without the need for leveling compound. I think you'd get it level enough with a length of wood and spirit level

How do I prepare this floor surface for tiling? It's the inside of a fireplace. Do I put in some sort of sand and cement mix, compact it then wet self levelling concrete?
