Well, I think I can make out a timing chain to the right, that connects both cranksets. Which in turn means there's got to be some double-hub-shenanigans going on on the rear, otherwise the whole thing won't move.

Yeah timing chain outside the triple chainrings up front but cranks are definitely out of sync. Could just be poor setup.
Don’t know about the double hub, looks like a normal hub to me with the stoker cranks bolted on the outside of the dropouts but don’t know how that’d work.

Right, hadn't spotted the out of sync pedals...
Double hub could be possible with the center hub around a large hollow axle, and the outer hub with a smaller solid axle running through the first one.

There's a tab which is secured by the QR/axle of which the bolts thread into.
Interested to know too since I'll be doing the same with mine.

Wish I had a photo of my disassembled/stored Koga Miyata Runner. Was a 105/DA setup on a 1994 frame, and my daily... til I got sick of car drivers and started riding mtb/bmx

Sounds cool - I always keep an eye out of Koga Miyatas in my size. My Simoncini project is coming together nicely. Gone a bit posher than anticipated in a few areas, but at least the frame looks like shit, to balance it all out :-)
Looking at the photo I don’t know that the pilot and stoker cranks are synced.
I’d thought the stoker cranks would be synced to the pilot cranks and then they would drive the rear wheel but who knows.
Also looks like the Q factor might be huge.
