I could well imagine it's got worse for him, but I don't watch him obsessively. For it to have got worse he would probably have had to be placed under more stress than before, when every indication is that most things don't really get to him, probably using wall-building techniques (not meant as a punning reference to his Mexico 'policy' but in a kind of Pink Floyd sense) he's had since childhood. I'm sure those around him are mostly concerned about somehow keeping the ship from sinking. That he might not be fit to govern would trigger a massive crisis.

Maybe, but I have to be honest, I really struggle to qualitatively compare his current bullshit with his bullshit from two or three years ago... I honestly couldn't tell you whether he's going downhill or not. Though there are reports by people around him that seem to suggest he's having more and more senior moments.
