details of what wheel might help too
chain tugs : well worth getting a pair, they just work so well holding the rear wheel in place

https://photos.app.goo.gl/MwwJt1yJmSUwfQDAASo I have one side with a chain tug and a washer and the other side with none.
Is the washer anything to worry about? I remember trying to use the chain tugs when I first got it and they were so fiddly I stopped bothering

You might want some "proper" track nuts (with captive washer).

Thanks everyone - very helpful
Never used to have this issue on my fixie but my chain loosens a bit as soon as I start cycling. Then after 3 or 4 days it's loose enough to come off after hitting a bump.
I notice I have a washer on one side of the rear wheel between the nut and the frame and not on the other.
Do I need a washer on both is this likely to be my issue? If so can anyone point me towards what I need to buy?
Thanks
