I also recommend a wool or silk liner. If really tough, pop them on the radiator/sit on them before you leave the house
But the arm warmer under the jersey/jacket has been a big find for me this winter

Seriously though I have the same issue (Raynaud's) and my best bet is lobster gloves worn loose as YAL mentioned to stop tightness cutting blood flow. Even if dexterity is reduced put them on when you are warm beforehand and don't take them off! Make sure your core is warm as well.
