I remember talking to some bloke who was talking about engineering solutions and problems they had with forklifts having their motors burnt out with people sticking them into reverse to stop. At considerable cost they tried, and failed, to design a control system that would prevent this from happening.
It turned out the solution was to tell the workers to stop fucking doing it or else it's a disciplinary offence.
