I've got a niner gravel fork which fits the bill. Not cheap though.

Take a look at pro-mance, they also can provide other models not shown on the site when you contact by email.
My forks are not here yet but I think

Surely just buy any of the myriad of flat mount models and use an flat>post adapter?
