Pedantry corner alert--as part of the mythology he was creating for Britain, Tolkien actually conceived of a mythological Britain as Tol Eressëa, the Lonely Isle. Middle-earth stood for the rest of Europe/Asia/Africa. The early conception was later dropped (why is rather complicated), but Britain was never seen as part of Middle-earth. (There was never a role in that attempted mythology for Ireland, north or south.)

Again, this is complicated in the development (ultimately unfinished and abandoned) of the mythology, but America is made allowance for as the 'New Lands' after the world is made round following the Fall of Númenor.

None of this helps explain the huge burning eye above my local post office.
