Seems to be unavailable now (also new model doesn't have the brace and is alloy) so second hand may be best bet, but the dmr wingbar meets most of these criteria

I already untwisted 3 forks by putting them in crappy frames. You find a friend to hold the crappyframe and you pull the fork towards you until it comes straight.
