When people say 'me cago en la leche', it's short for 'me cago en la leche de tu puta madre' -you're not shitting in your own milk.

But, to be fair, me cago en la pinga is a very Cuban expression that I hadn't heard before, so maybe he is indeed shitting on his own dick.
