If the first item is as the result of clicking through then the whole cart gets acknowledged.

Is this true for all retailers, not just amazon?

No, it varies.

And the rates vary.

I've basically disabled some systems in the because the rates were appalling (1% on linked item sale, less than 1% on remainder of cart for eligible items).

Amazon are pretty good, as are Ebay... but with everyone else it varies.
