If the key isn't loose, then the clamp must be. If the clamp isn't loose, then the key must be. There's a reason why a proper Major Taylor stem has a square extension, you bell ends.
Oh, and you did all that custom work but you couldn't be arsed to make some spacers which actually line up properly with the steerer clamp? Poor, very poor.
