Caulking - what should I be using, and do I just fill around the bits highlighted?

Frame sealant, lots of types available, different colours or clear. If you find an acrylic one you can paint it, silicone will repel paint. If you use masking tape either side of the joint it will save you smearing it all over the mortar/woodwork. If you're painting the door afterwards you could try Zinnser 1 coat exterior gloss to make the job easier. I use 2 coats of it generally and it's held up well over the last few years, gloss is easier to clean and lasts longer.

Thanks. I was *kinda* expecting that to be the answer, but was second-guessing myself, as I thought "if it's that easy, why tf didn't they do it when they built the extension, and why has nobody done it in the 10 years since?"
