I don't get it.
The outer is slightly thicker than normal, and slightly more hollow inside, allowing a thin sleeve to slide in all the way from the shifter to the derailleur (the white sleeves as pictured)
That makes more sense! So it still has an outer, then it has a fully sealed 'middle' and then there's the inner. Pictures are worth 1000 words.
