In the UK at least, anybody involved with the book (including printers and distributors etc) can be sued for defamation along with the publisher or author so getting it removed from sale isn't as unlikely as it sounds. WH Smith wouldn't stock Private Eye for many years.
Also - Cancellara would have to sue in every country the material has been published. A defamation award in the UK carries zero weight in France for example.
