Surly 24 rack. But any rack can come loose at the crown and rotate and block the wheel.
It has happened to me as well. I was a lot more lucky than Scooter tho.
Regular bolt tension checks + locktite can provide sense of security. I have even thought about a small safety wire, just in case. -

Glad you're on the mend, Jack. That could have been so much worse.
You say you're already getting bits of memory back. I think if your total memory loss was only one week, you may recover most of it. I've certainly heard of cases where that happened. As you say, the drab week may not be worth it, but it would be worth having a memory of the moments before the crash, especially if there were really no witnesses.
I take it that it must have happened at the speed hump right outside the Tufnell Park Primary School gate? As you say, a dooring is certainly possible, but it's probably wise to be satisfied with the simpler explanation and not speculate. You don't say if your wheel was damaged, though--I really have no idea, but I'd expect it to have been shredded at least a bit if the rack got in there? Then again, perhaps it's precisely indicative of the sudden stopping force generated if it wasn't.

Maybe better to cross drill it and put a pin through it? I quite like the design of the Soma porteur (and all the other racks similar to it) that uses a slot for mounting to the crown. Even if your bolt lets the strip slip it won't be able to rotate into the wheel and you can (quickly) stop and tighten.

Just seen this thread. So pleased that you are recovering and hope the recovery continues swiftly and fully.
Would suggest speaking to a cycle injury solicitor soon and not to seek to repair bike until you have done so. There may be evidence in the bike that would help establish if the accident was the fault of the rack manufacturer.

Pizza rack? Hmm, seems that regular checking for loose bits is the best way to avoid eating a surprise shit.
Big up @spotter glad to hear you're alright (relatively) and the bike has been found.
Sounds as though the Soma rack mounts on @spotters buckled and met the tire at the crown and @hulsroy yours went forward?
I guess to avoid it going forward you could use a wire crimped loop around the fork crown / headtube ... for security.

I haven't read through the whole thread so I may be repeating something already mentioned.
Have you considered getting someone (on here) to canvass the school and residents in the area to establish if there is any CCTV that might put your mind at rest as to what exactly happened and if anyone else was involved. -
