Maybe better to cross drill it and put a pin through it? I quite like the design of the Soma porteur (and all the other racks similar to it) that uses a slot for mounting to the crown. Even if your bolt lets the strip slip it won't be able to rotate into the wheel and you can (quickly) stop and tighten.
Surly 24 rack. But any rack can come loose at the crown and rotate and block the wheel.
It has happened to me as well. I was a lot more lucky than Scooter tho.
Regular bolt tension checks + locktite can provide sense of security. I have even thought about a small safety wire, just in case.
