Plenty of other options available. Bolt on cogs. Single speed disc hubs with 6 bolt cog on the left.

Does this actually work? Could I have a cassette on one side and a fixed cog on the other?

Considering it can handle braking forces I imagine it would be fine with skids/high-torque/etc.

Nothing wrong with riding 6 bolt fixed. However, finding a hub that with a cassette and 6 bolt disc mount that will fit your rim brake bike won't be easy..
