Oh no, how terribly sad and how utterly avoidable that sounds even from the initial reports.
The report indicates that it happened at the Camden Road/Brecknock Road/York Way junction. Did you see it differently?
There are all sorts of factors that may have played a role, pointless to speculate at this stage until more information comes out.
