Trump clearly has no idea what he's doing, but his default in any situation is to escalate.
He can't pre-empt, he can only react to a Nork launch by hitting Pyongyang whilst trying to intercept the Nork missiles, which the multi-layered US defence (from some quick Googling) looks like it may be equal too - but where else might Kim target?

How much of this is true?
https://www.google.co.uk/amp/s/amp.theguardian.com/commentisfree/2017/aug/07/whats-worse-than-war-with-north-korea-chinaThe bit about any nuclear launch would then provoke China into attacking north Korea was interesting.

Seoul. It would be a major propaganda win for Kim to capture Seoul. Any war he engages in will be mostly land based from the NK side and with Seoul just 35 miles from the border it would make sense for him to do a lightening strike before the US can get effective boots on the ground to stop him.

Listening to Trump talk about "fire and fury like the world has never seen"...

Also, no way do NK have guidance systems that can target Guam, even if their missiles made it that far in the first place.
Usually when the North escalates rhetoric, it's really a round-about way of asking for aid/assistance from China for something. Talking shit back doesn't really do anything other than make it louder. Trump could easily just be trolling them for fun at this point as he obviously knows their actual capabilities.
Also, a quick strike at NK, taking out the leadership and key military stuff would just cause a massive humanitarian crisis that China and South Korea want to avoid at all costs.

Whats the confidence level on the "they're guidance is shit" part?

It's all a bit.

a quick strike at NK
If it were Obama, or Bush, or Clinton, or Bush, or any other US president, you could reasonably expect this, should the circumstances absolutely demand it, with the appropriate backing (either explicitly or implicitly).
With Kim Jong Trump, who knows? A big strike, the biggest strike, we have nukular, the best nukalar, you see, and my friends, I have lots of friends, they tell me, the nukular, you better believe it.
