Very, very hard to hit the head. Even if you are told to aim for it, especially with iron sights + lots of adrenaline. I'm sure the officees were well trained but they are human.
With semi-auto weapons they are usually taught to shoot in bursts of three. I would hazard a guess that many of them would have been covering fire to allow colleagues to get in closer.
As I recall from an eye witness Statement yesterday it was a member of the public who directed a gun car to the correct location. Four or so shots and they were all down.
