I see what you mean, with the thumb being 'neath the bar.
However, historically thumb shifters have almost always been attached as that one, probably because not only can you push it further without the web between the thumb and index finger getting in the way, but it's also easier to nudge back with your index finger knuckle. -

exactly. I just fitted thumbies and ended up rotating them to about 60 degrees off horizontal.
