I've had the barrel break of twice in the past couple of years. These things aren't going to last forever. I've not just gotten more paranoid about the creaks when breaking as that is the only thing I can tie back to both incidents as possible warnings. I am guessing the strands of the cable were starting to work themselves loose over time until a point where an emergency stop was enough to kill it off.
