The vet took a look at where he had been chewing and there was nothing obviously causing it. She said she wouldn't need the muzzle to do his glands but Mrs S insisted for her own safety. It turns out he really wasn't keen and in his haste to turn and have a go it increased the amount of squeeze used by the vet. That and the alteration of the angle of attack caused "the contents" to shoot up her arm/face*. She was ready for the second one but declared that all was well in that department as it turned out. It would seem he just had a bit of a chew and has started a vicious circle.
*Worth the fee in itself from what I've heard.
It's not worth spending money on the vet to do this it's pretty straight forward.
