Right, I reckon a Gavanelle diy job is definitely a possibility using one of these Sturmey thumbshifters.
The Sturmey has a bit of adjustability built in and I think the Gavanelle does too and it'd be difficult to retain this but is it really necessary?
Cut a hole in the lever in this shape
Insert this part from inside the lever, place the black bit with the cable stop over and then attach the shifter of your choice?

Im not sure, I think they might be older Honjo when they did that single point support strut. Velo Orange Zeppelins are pretty much the same though, and easier to get.
