1 - sounds like it might be PTSD. It's a normal reaction, but you might want to consider counselling. *
2 - ask your GP for a physio referral. You don't want to be taking random internet advice that could cause further damage.
3 - Most councils have some sort of reporting mechanism for problems on roads. They should also have policies in place for maintaining / checking roads, to make sure that they are in a suitable condition, including for people on bicycles. If you have legal cover (through home / other insurance), you could ask them to set a solicitor onto it.
