I have the exact problem with every single pair of cycling shoes I've ever bought.
Empire SLX are by far the narrowest and caused me the most pain in this department.
Unfortunately it's just something that I've come to accept - that bit of my foot gets fucked if I ride 120km+ for more than about 4 days on a row.
The SWorks 6 are the only shoe I've had in a long time which allow me the pleasure of over 2 days riding before that part of my foot is in a lot of pain.
I religiously ice the fuck out of it after a ride if it's starting to give me shit, but generally, now I don't feel anything unless it's back to back long days in the saddle.
Some days I just want to take a Stanley knife to it.....I feel your pain

@dick are the empires any wider? The problem I have it that I have normal with feet, apart from the base of my 5th metatarsal (i.e. my little toes bottom joint) sticks out.
The toe box is a good width its just the mid section.
