That's the thing, while says your chain is at 0.5 (using Park Tools chain wear tool), sometime that mean 1 or 2 out of 10 (hypothetically) cog is worn.
While that may not seemed like a good reason to change cassette, they may be the gear you use the most, and can accelerate the wear of your chain, and thus the cassette too, even chain skipping occurred.
In my experience, it is worth replacing both to ensure 100% drivetrain effectively.
Having said that, I usually replace my chain before they wear out too (and using the Rohloff tool), as cassette aren't cheap.

Will a 6800 rear mech be ok to use in a 1x setup (with a narrow-wide ring at the front)? Or is it a better idea to go for a dedicated 1x rear mech with clutch? For road only use.

It'll be fine. I've been using one on my cross bike for the past 2 years without any issues.

Have used 5800 short and medium. 6800, 6870 short and medium all 1x (narrow wide chainring) without dropping a chain on road,tt and cc bikes

pretty much set on buying a tiagra 4700 groupset for my winter bike. Do I need to worry about BB size etc, or can I use the one that is one there at the mo? Currently running Sora 9 speed (not the latest one) with an external BB on a Genesis Equilibrium - it's a shimano BB but I can't remember what spec...

As long the thread is the same should be the safe.
Actually Never heard of them exist ed. That would be great to have though.
My thinking is based on the cassette should outwear several chain before need replacing.
